Headache Specialists of Oklahoma is ‘in network’ with most major insurance plans! Please view our ‘financial information’ page to learn about all insurances that we accept.

For those insurances that we do not accept, we do provide direct pay services with transparent fees (see below) so that you are still able to receive care from Dr. Duvall. Full payment is due at the time of service. You are still able to file a health insurance claim with your current insurance, and insurance typically reimburses 50-80% of the services we provide.

We are often able to provide same day or next day appointments, virtual consultations, and more.

  • NEW PATIENT CONSULTATION

    $300 NEW PATIENT.

    $200 ESTABLISHED NEW PATIENT, PREVIOUSLY SEEN BY DR. DUVALL IN PRIOR PRACTICE.

  • FOLLOW-UP APPOINTMENT

    IN OFFICE FOLLOW-UP $200 *appropriate for continued evaluation and management of ongoing headaches.

    TELEMEDICINE FOLLOW-UP $125 *appropriate for patients who have established a medication regimen that is working well and need to renew prescriptions, or who have brief follow-up questions.

    *Please note established patients are required to be seen for follow-up appointment at a minimum of every 6 months.

  • PROCEDURES

    BOTOX INJECTION FEE $300 plus drug cost. We will authorize drug cost with insurance if applicable.

    NERVE BLOCK AND TRIGGER POINT INJECTIONS $450 total for all injection sites (up to 4 total trigger point injections).

    SPG (sphenopalatine ganglion) BLOCKS $100 plus device cost.

  • OTHER

    CANCELLATION/NO SHOW $25 if cancellation is LESS THAN 48 HOURS PRIOR TO APPOINTMENT TIME (or after 11 am on Fridays for a Monday appointment).

    CANCELLATION/NO SHOW $50 if cancellation is 24 HOURS PRIOR TO APPOINTMENT TIME, OR NO-SHOW APPOINTMENT WITHOUT CANCELLATION.

    PRIOR AUTHORIZATIONS FOR MEDICATIONS $25 per medication/injection/treatment per year.. Certain medications and procedures require prior authorizations to be complete. This process can take hours of our time and fees for this service support our administrative staff.

    MEDICAL RECORD REQUESTS $25 for records less than 50 pages, billed per page $0.30 thereafter. X-rays, imaging and other media $5 per page. Max fee $200.
